Recently, our church requested and received funding for a project we pursued to impact lostness. Below you will find our written report of how the funds were used to complete the project. You will also find our evaluation of how the project went and a report of how we have followed up since completing the project.

 

We understand that the completion of this form is "part of the bargain" since we want the association churches to know that their funds have been handled by us in an appropriate way.

 

We also understand that no subsequent funding requests can be considered unless/until this follow-up information is returned to the Impacting Lostness team.
